- Coverage is available to groups of 2 to 9 active full-time
employees, working a minimum of 30 hours per week on a regular
basis
- All eligible employees and eligible dependents may be covered
upon completion of the specified waiting period.
- Employer chooses maximum benefit amounts and options, such
as elimination and waiting periods and benefit durations.
- Certain industries may not be eligible for one or more coverage
(except in the state of Michigan)
- Non-Contributory plans require 100% of all eligible employees to
be covered
